US Commuter Players Application
USAH-HC Tryout Notice Form

The Alliance and OHF have sent feedback on poor quality forms received.  They alerted us the USA Commuter Players, PTT scans have to be clear enough to read or they won`t be reviewed.  Please fill in written as boldly as possible and good size lettering (preferably dark/black ink).  Every time it gets printed to sign and re-scan quality seems to get poorer, especially initial information filled in (usually player info and signatures) tends to end up barely legible by final copy.  Also jpg photos of documents forwarded often are not taken squarely and distorted, not flat looking page, uneven shades and some text thick but not sharp and some faint. Quality scans are preferred.  If you choose to take jpg photo (with a cell phone) be 100% certain you are square to page with even lighting and finished quality is as close to original as possible.

US Players

Please fully complete all US Player Tryout Forms. Any gaps in information, signatures or dates will result in rejection by US and HC Authorizing Associations. Please note electronic signatures are not accepted. It is important to process these ASAP to complete approvals prior to Tryouts. There is also an available appeals process (which takes extra time). No participation is allowed without completed and approved forms.
